Acupuncture vs Chiropractor in San Francisco

For back or neck pain, acupuncture and chiropractic are both options. A chiropractor may suit you if you want hands-on treatment and movement advice; acupuncture may suit you if you prefer needling or want pain care alongside treatment for sleep or other symptoms. The diagnosis, your preferences and your response to treatment matter more than choosing a profession once and for all.

Trained and practised in Beijing Fewer needles Modern acupuncture

Dr Deb Ma, PhD, LAc, OMD  ·  1640 Valencia St, San Francisco

17 years, most of them in BeijingPhD from Beijing University of Chinese Medicine, then clinical practice at Beijing United Family Hospital.
Integrated practiceTen of those years working across departments with neurologists, OB/GYNs and psychologists on the same patients.
Fewer needlesPoint selection is deliberate and minimal. Most patients are surprised how little is used.

Chiropractic centres on movement and manual care; acupuncture uses needles

Both clinicians assess pain and function. Chiropractic care can include exercise and advice as well as adjustments; an acupuncture visit can also address symptoms beyond the painful joint.

AcupunctureChiropractic
What it is Fine needles placed at specific points on the body, usually left in for 10 to 30 minutes. Manual treatment of joints and muscles, sometimes including a quick, controlled thrust called an adjustment. Care may also include gentler mobilisation, exercise and advice. An adjustment does not have to make a popping sound.
What it treats Back, neck and shoulder pain, headaches and sports injuries, alongside sleep, digestion, mood and menstrual concerns. Primarily musculoskeletal problems, including back and neck pain. The evidence and treatment options differ by condition.
A first visit 75 minutes at Puzzle for your history, examination and treatment. Follow-ups are 60 minutes. Dr Deb agrees a treatment course with you and reassesses how pain and daily activity change. History and examination, which may include movement, strength and reflexes. The clinician decides whether treatment, further investigation or a referral is appropriate. Visit length and treatment schedules vary by practice.
Licence Licensed acupuncturist (LAc), through the California Acupuncture Board. Dr Deb also holds a PhD from Beijing University of Chinese Medicine. Doctor of Chiropractic (DC), through the California Board of Chiropractic Examiners.

Trials support both approaches for some pain conditions, without naming an overall winner

A trial of acupuncture against sham treatment cannot tell you whether it works better than chiropractic. The condition and the comparison used in the study matter.

A 2024 trial studied chronic sciatica caused by a herniated disc. Among 216 people included in the analysis, leg pain fell by about 31 points on a 0 to 100 scale after four weeks of acupuncture, compared with 15 points after sham acupuncture.

Tu et al., JAMA Internal Medicine, 2024. Of 220 people randomised, 216 were included in the analysis. Treatment was ten sessions over four weeks, and a difference remained at one year. No serious adverse events occurred in this trial.

This supports acupuncture as an option for this particular kind of sciatica. It does not establish that acupuncture beats chiropractic or that everyone with pain down a leg needs the same treatment.

For low back pain, spinal manipulation can offer modest improvements in pain and function, with results often similar to other recommended treatments. Research also supports manual care for some neck pain, though certainty varies. A chiropractor who combines appropriate hands-on care with exercise may be a good fit if restoring comfortable movement is your main goal.

Persistent or worsening symptoms deserve reassessment rather than an open-ended course of either treatment. New bladder or bowel changes, numbness around the groin or worsening leg weakness need urgent medical assessment. Surgery is a separate decision based on the diagnosis and severity, not an automatic next step after a fixed number of weeks.

At Puzzle, Dr Deb may also use cheek acupuncture or FSN needling, a shallow needling technique used around tight muscles. These are distinct techniques; the sciatica trial above did not test them.

Choose care around the problem, and reassess what changes

At Beijing United Family Hospital, Dr Deb worked with specialists in orthopedics, neurology and rehabilitation medicine. In San Francisco, she coordinates with a patient's existing clinicians. If you use both acupuncture and chiropractic, agree on the goal and tell each clinician how you respond between visits.

Pain relief and better movement are useful goals for either approach

The problemUsual first stopWhat the other addsWhat to tell each clinician
Sudden low back pain after lifting, no leg pain Either may be reasonable after an assessment. Choose a clinician who explains the plan and helps you stay active. A different way to manage pain if the initial approach is not helping. There is no required order. How it started and which movements or daily tasks are difficult.
Low back pain with pain down the leg Assessment of the leg symptoms first. Acupuncture has trial evidence for chronic sciatica from a herniated disc. Movement advice or another approach to pain care, depending on the examination and response. Pain, numbness or weakness and whether they are changing. New bladder or bowel problems require urgent medical care.
Neck pain with headaches from the neck A clinician who can assess whether the headache is coming from the neck. Chiropractic care may include mobilisation and exercise; acupuncture is another pain-care option. An alternative if the first approach is not helping. Discuss the benefits and risks of any proposed neck manipulation. Headache frequency, triggers and any change in the usual pattern.
Frozen shoulder (a stiff, painful shoulder that has lost its range) An assessment and a shoulder rehabilitation plan. Neither profession is an automatic first choice based on this label alone. Acupuncture may be considered for pain alongside appropriate movement work. Manual care should be matched to the shoulder's irritability. How far the shoulder moves at each visit, so both work from the same number.

These are starting points for a clinical discussion, not results from a head-to-head trial of acupuncture and chiropractic.

Frequently Asked Questions

Can I see a chiropractor and Dr Deb in the same week?

Yes, when both clinicians agree the treatments fit your condition. Tell each one what the other is doing and report any soreness or change in symptoms. Dr Deb can coordinate with your chiropractor so the plan and timing are clear.

Does my chiropractor need to know I am getting acupuncture?

Yes, and Dr Deb is glad to write to them directly. The two of them are working on the same joints and muscles. Bring any X-ray or MRI report your chiropractor has ordered.

Can a chiropractor do acupuncture or dry needling in California?

Not on a chiropractic licence alone. California's rules say the licence does not cover breaking the skin, so needling is outside it. A chiropractor who also holds a separate acupuncture licence is a different case.

Does acupuncture hurt compared with a chiropractic adjustment?

Less than most people expect. Acupuncture can cause a brief prick, an ache or a heavy sensation; tell Dr Deb if anything feels sharp or uncomfortable. Chiropractic techniques vary from gentle mobilisation to a quick thrust. Either treatment can leave temporary soreness.

What does it cost, and does insurance cover it?

Puzzle does not bill insurance. The consult is free, a new patient visit is $255 for 75 minutes, and follow-ups are $205 for 60 minutes. You receive a receipt to submit for any out-of-network benefits. Chiropractic coverage varies by plan; Medicare covers certain spinal manipulation services, not every service a chiropractor provides. Check your benefits and the provider's fees before starting.

Bring the other clinician's notes

Two readings of the same problem are better than one.

The consult is free. Dr Deb will go through your history, what your chiropractor has found, and where acupuncture might fit alongside it.
